JOB TITLE: MEDICAL SOCIAL WORKER
RESPONSIBLE TO: NURSING SUPERVISOR


Job Summary: The Medical Social Worker is primarily responsible in identifying the related social needs of the patients and providing the services to meet these needs. The Medical Social Worker deals with the patient's attitudes and reactions to illness as they affect his capacity to make positive use of medical care.
Minimum Qualifications:
Graduate of a school of social work accredited by the Council on Social Work Education.
Applicant must meet or exceed minimum qualifications for each of the following background checks:  Criminal Background Investigation, Professional and Personal Reference Checks, and a pre-employment physical and drug/alcohol screen. 
Minimum of one year of experience as a full time social worker in a health care setting and/or working with disabled, geriatric population
Current TB Clearance
Solid communication skills (bilingual in Spanish and English a plus)
Must be genuinely concerned about helping people and have high moral standards of honesty and integrity.
 
Essential Functions:
Provides a variety of client care duties including but not limited to:
 
A. Patient Care Management
Provide direct aid to patient and family: Make an initial psychosocial assessment of patient; Identify those psychosocial needs that may affect patient's ability to freely utilize medical treatment plan; Obtain financial aid and information for patient's need of such services.
Maintain records of social services performed and submit weekly services performed on each visit.
Participate in the development of plan of care.
Observing, recording, and reporting, information on the patient's condition to the attending physician and in the patient's health record.
 
B. Coordination of Services
Act as liaison between patient, family and community resources:  Make direct referrals to community agencies when appropriate;  Placement in other facilities as necessary, i.e. skilled nursing facilities, board and care; Discharge planning with patient, family and staff to maximize medical and social resources to facilitate return to optimum level of functioning.
Maintain communication with Director of Patient Care Services regarding special situations of patients.
Participate in discharge planning
 
C. Staff Development
Participate in patient case conference, team and staff meetings.
Provide in-service and consultation to staff.
Community Responsibility
Act as a representative of the Agency in various settings to facilitate cooperation and communication between Agency and community.
